Occupational Therapy assessments for Home Care Packages
We provide comprehensive Occupational Therapy (OT) assessments for older adults with a Home Care Packages, focusing on maximizing independence, safety and quality of life at home.
What do we assess?
We assess you within your home, to determine what modifications or equipment may be required. We assess your ability to complete the following:
- Self-care tasks , such as showering, toileting and taking medication.
- Domestic tasks, such as meal prep and cleaning.
- Mobility, within the home and in the community.
- Home safety.
What recommendations do we make?
Our aim is to ensure that you remain safe within your home, and continue completing tasks as independently as possible.
We recommend minor home modifications, equipment, and discuss ways to complete tasks safer and easier.
Who do we assess?
We can only supports those who currently have a My Aged Care package, or those privately funding this assessment.
Unfortunately we are not registered under CHSP and cannot assist if you have been provided with a referral code.
