What happens to my information?

We collect, use, disclose and secure your personal information in accordance to the Privacy Act 1988, the NSW Health Records and Information Privacy Act 2002, ACT Health Records (Privacy and Access) Act 1997 and the NDIS Act 2013. 11. We will not use or share your personal information with anyone without your consent, unless we have concerns for your safety or we are required or authorised to by law. For example Mandatory Reporting of child protection concerns.

What area do we service?

Our Occupational Therapy services are based in Dapto, NSW, however we service the whole of Illawarra. This encompasses the two cities of Wollongong, Shellharbour, and the coastal country town of Kiama.

Where does the assessment take place?

We organise to complete the assessment in the most appropriate environment for your needs. This means, that we will come to your home or your workplace. Completing the assessment at your home or workplace means that we can clearly see what areas of issues you may need assistance with.

What do I need to have available at the assessment?

The Occupational Therapist will bring the assessment documents and any relevant tools to complete the assessment. It is always good if you can have your current medical documents and current NDIS plan available. This information is relevant to your assessment and will assist in ensuring the correct information is being recorded.

How long does an assessment take?

An Occupational Therapy assessment can take anywhere between 1-3 hours. The time depends on your needs and goals, and the type of assessment you require. Don’t forget, that we are here to help! So, if you need to have a break within the assessment, or pause the assessment and resumed at a later time, we are more than happy to assist. Let your Occupational Therapist know of your needs.

What if I need to cancel or reschedule my appointment?

Please contact us, no later than 48 hours prior to your appointment for any cancellation. There is a cancellation fee if contact has been made within 48 hours of the scheduled assessment. We will always try to reschedule your appointment within the same week, to avoid such fees.

How do I pay for my appointment?

An invoice will be sent to either your Support Coordinator or yourself, for payment prior to receiving your Occupational Therapy report. If you have access to NDIS funds, you can utilise your ‘Improved Daily Living’ funding, under Capacity Building.

Send through your medical documents, and NDIS plan for us to review.




We will arrange an assessment with you, either at your home, work or over videocall.




A report of our assessment will be completed and sent to you, and your coordinator (if required).
